Ce formulaire permet à un mandataire de suspendre une procuration existante conférée par une institution (MRS, MRPA, CSJ) dans le cadre de l’application Facturation du tiers payant par le biais de l’application CIN « MyCareNet ».
Met dit formulier kan een volmachthouder een bestaande volmacht, gegeven door een instelling (ROB, RVT, CDV) in het kader van de toepassing facturatie via de NIC-toepassing “MyCareNet” stopzetten.
