Vu le risque plus élevé de complications hémorragiques avec les thiénopyridines par rapport à l’acide acétylsalicylique, il est en principe recommandé d’interrompre un traitement par le clopidogrel ou le prasugrel 7 jours avant une intervention chirurgicale, et par la ticlopidine 10 jours avant l’intervention.
Gezien het hogere risico van bloedingscomplicaties met de thiënopyridines dan met acetylsalicyzluur, wordt in principe aangeraden een behandeling met clopidogrel of prasugrel 7 dagen vóór een heelkundige ingreep te stoppen en een behandeling met ticlopidine 10 dagen vóór de ingreep.

Dans une mise à jour de la revue Cochrane ayant comparé les thiénopyridines (clopidogrel, ticlopidine) en monothérapie à l’acide acétylsalicylique chez des patients à risque cardiovasculaire élevé, une analyse de sous-groupe a été effectuée chez les patients ayant déjà eu un accident vasculaire cérébral ou un accident ischémique transitoire.
In een update van de Cochrane review waarin thiënopyridines (clopidogrel, ticlopidine) in monotherapie werden vergeleken met acetylsalicylzuur bij cardiovasculaire hoogrisicopatiënten werd een subgroepanalyse uitgevoerd bij patiënten die reeds een cerebrovasculair accident of transient ischaemic attack doormaakten.

Compte tenu de l’effet inhibiteur de l’iloprost sur les fonctions plaquettaires, son association avec des anticoagulants (comme l’héparine ou les coumariniques) ou d’autres inhibiteurs de l’agrégation plaquettaire (comme l’acide acétylsalicylique, les anti-inflammatoires non stéroïdiens, la ticlopidine, le clopidogrel et les antagonistes de la glycoprotéine IIb/IIIa tels que abciximab, eptifibatide et tirofiban) est susceptible de majorer le risque hémorragique.
Omdat iloprost de trombocytenfunctie remt, kan het gelijktijdig gebruik van anticoagulantia (zoals heparine, coumarine-anticoagulantia) of andere trombocytenaggregatieremmers (zoals acetylsalicylzuur, niet-steroïde anti-inflammatoire geneesmiddelen, ticlopidine, clopidogrel en glycoproteïne IIb/IIIa-antagonisten: abciximab, eptifibatide en tirofiban) het risico op bloedingen vergroten.

– Les médicaments comme le clopidogrel, la ticlopidine, la ciclosporine, la kinine. peuvent occasionner une microangiopathie thrombotique (comme en cas de purpura thrombotique thrombocytopénique), pouvant à son tour causer une atteinte rénale.
– Geneesmiddelen zoals clopidogrel, ticlopidine, cyclosporine, kinine, . kunnen thrombotische micro-angiopathie veroorzaken (zoals bij thrombotische thrombocytopenische purpura) waardoor nierschade kan optreden.

Associations non recommandées : héparines non fractionnées et dérivés de l’héparine, héparines de bas poids moléculaire (HBPM), fondaparinux, désirudine, thrombolytiques, antagonistes des récepteurs GPIIb/IIIa, clopidogrel, ticlopidine, dextran, sulfinpyrazone et antivitamines K. En raison du risque hémorragique, une surveillance étroite des signes de saignement est recommandée, notamment avec les AINS à demi-vie d’élimination supérieure à 12 heures
Niet aanbevolen associaties: niet-gefractioneerde heparines, heparine, heparines met een laag moleculair gewicht (LMWH), en heparinederivaten (fondaparinux, desuridine), trombolytische middelen, GPIIb/ IIIareceptorantagonisten, clopidogrel, ticlopidine, dextran, sulfinpyrazon en vitamine K-antagonisten. Vanwege het risico van bloedingen, in het bijzonder bij NSAID’s met een eliminatiehalfwaardetijd van > 12 uur, wordt daarom aanbevolen om nauwkeurig te controleren op symptomen die op een bloeding wijzen.
