Ze zijn vrijwel onveranderlijk verontrustend en de betrokkene tracht dikwijls, zonder succes, zich ertegen te verzetten. Ze worden evenwel herkend als zijn of haar eigen gedachten, ondanks hun onwillekeurige en vaak weerzinwekkende aard. Dwanghandelingen of rituelen zijn stereotiepe gedragingen die steeds maar weer worden herhaald. Ze zijn niet aangenaam op zichzelf en leiden ook niet tot de voltooiing van op zichzelf nuttige taken.
