1° trombocytose > 600 x 10/l na 3 maanden hydroxycarbamide bij een dosis van minstens 2 g/dag (2,5 g voor een lichaamsgewicht > 80 kg);
1° thrombocytose > 600 x 10/l après 3 mois d'hydroxycarbamide à une dose d'au moins 2 g/jour (2,5 g pour un poids corporel > 80 kg);

2° trombocytose > 400 x 10/l en witte bloedlichaampjes < 2500 ongeacht de dosis hydroxycarbamide;
2° thrombocytose > 400 x 10/l et globules blancs < 2500 quelque soit la dose d'hydroxycarbamide;

a) De specialiteit wordt terugbetaald indien ze wordt toegediend voor de verlaging van het verhoogde aantal plaatjes bij de patiënten die lijden aan essentiële trombocytose en waarvoor de aanwezigheid van een JAK-2-mutatie en/of de afwezigheid van het Philadelphia-chromosoom en/of de afwezigheid van een BCR-ABL-verschuiving bevestigd zijn geweest voor zover dat :
a) La spécialité est remboursée si elle est administrée pour la réduction du nombre élevé de plaquettes chez les patients à risque atteints de thrombocytémie essentielle et pour lesquels la présence d'une mutation de type JAK-2 et/ou l'absence du chromosome Philadelphie et/ou l'absence un réarrangement BCR-ABL a été confirméepour autant que :

de patiënt minstens éénmaal in het verleden een gecontroleerde trombocytose > 1 000 x 10/l heeft doorgemaakt
le patient ait présenté une thrombocytose contrôlée au moins une fois > 1 000 x 10/l dans le passé,

de patiënt een trombocytose > 450 x 10/l met een gedocumenteerd antecedent van trombohaemorrhagie in het verleden heeft doorgemaakt
le patient ait présenté une thrombocytose > 450 x 10/l dans le passé avec un antécédent documenté de thrombohémorragie
