De vergoeding kan toegestaan worden voor maximaal zes toedieningen (1/dag) KEPIVANCE 6,25 mg, aan 60 microgram/kg/dag, en voor zover dat de myelo-ablatieve conditionnering gebeurt ofwel met een totale lichaamsbestraling, ofwel met melphalan 200 mg/m, ofwel met BEAM (Carmustine 300 mg/m, etoposide groter of gelijk aan 400 mg/m, cytarabine groter of gelijk aan 800 mg/m, melphalan 140 mg/m) of met busulfan groter of gelijk aan 4 mg/kg/dag gedurende 4 opeenvolgende dagen en cyclofosfamide groter of gelijk aan 50 mg/kg/dag gedurende 4 opeenvolgende dagen.
Le remboursement peut être autorisé pour un maximum de six administrations (1/jour) de KEPIVANCE 6,25 mg, à raison de 60 microgrammes/kg/jour, et pour autant que le traitement myéloablatif soit effectué par irradiation corporelle totale, ou par melphalan 200 mg/m, ou par BEAM (Carmustine 300 mg/m, étoposide supérieur ou égal à 400 mg/m, cytarabine supérieur ou égal à 800 mg/m, melphalan 140 mg/m) ou par busulfan supérieur ou égal à 4 mg/kg/jour sur 4 jours consécutifs et cyclophosphamide supérieur ou égal à 50 mg/jour sur 4 jours consécutifs.
