41. begrijpt dat de Commissie zorg- en behandelingsstelsels rendabeler wil maken; benadrukt echter dat de voortdurende toename van de algehele kosten voor gezondheidszorg en sociale steun niet uitsluitend aan de vergrijzing kan worden geweten; beseft dat de toename van het aantal chronische aandoeningen een belangrijke rol speelt in de stijgende kosten van gezondheidszorg en sociale ondersteuning; tekent daar echter bij aan dat, als de kosten van de gezondheidszorg steeds meer gedragen door de individuele personen, er waarschijnlijk een vicieuze cirkel zal ontstaan waarin de gezondheid en het welzijn van mensen in het gedrang komen omdat zij dan wellicht gedwongen worden hun vaak beperkte middelen op een andere manier te gebruiken of om medische behandeling, bijstand en goede voeding uit te stellen of te stoppen of er zelfs helemaal van af te zien, waardoor de kosten van gezondheidszorg en sociale ondersteuning uiteindelijk steeds hoger zullen worden, zowel voor het individu als voor de maatschappij;
41. Acknowledges the Commission’s intention to contribute to making care and cure systems more cost-efficient; emphasises, however, that the continuous increase in overall healthcare and social support costs cannot be attributed to an ageing population alone; acknowledges that the increasing prevalence of chronic diseases play a substantial role in rising healthcare and social support costs, however, notes that where the burden of healthcare costs is increasingly borne by the individual this is likely to create a vicious circle whereby people’s health and wellbeing are put at risk as they might be compelled to reallocate their often limited resources or when they have to postpone, abandon or even refrain from treatment, assistance and adequate nutrition, which in turn might lead to ever greater healthcare and social support costs both to the individual and to the society;

De lidstaten kunnen algemene voorwaarden, behandelingscriteria en wettelijke en administratieve formaliteiten inzake ontvangst van gezondheidszorg en terugbetaling van kosten van gezondheidszorg blijven toepassen, zoals de vereiste dat de patiënten een huisarts raadplegen alvorens een specialist te consulteren of ziekenhuiszorg te ontvangen, ook jegens patiënten die gezondheidszorg in een andere lidstaat verlangen, mits deze voorwaarden noodzakelijk zijn, in redelijke verhouding staan tot het doel, niet discretionair of discriminerend zijn.
Member States may maintain general conditions, criteria for eligibility and regulatory and administrative formalities for receipt of healthcare and reimbursement of healthcare costs, such as the requirement to consult a general practitioner before consulting a specialist or before receiving hospital care, also in relation to patients seeking healthcare in another Member State, provided that such conditions are necessary, proportionate to the aim, not discretionary or discriminatory.

Overeenkomstig de in de rechtspraak van het Hof van Justitie vastgestelde beginselen en zonder afbreuk te doen aan het financiële evenwicht van de stelsels van gezondheidszorg en sociale zekerheid van de lidstaten, dient te worden gezorgd voor een grotere rechtszekerheid ten aanzien van de terugbetaling van kosten voor gezondheidszorg, zowel voor de patiënten als voor de gezondheidswerkers, zorgaanbieders en socialezekerheidsinstellingen.
In accordance with the principles established by the Court of Justice, and without endangering the financial balance of Member States’ healthcare and social security systems, greater legal certainty as regards the reimbursement of healthcare costs should be provided for patients and for health professionals, healthcare providers and social security institutions.

Deze richtlijn heeft niet tot doel patiënten een aanspraak te geven op terugbetaling van de kosten van gezondheidszorg die verleend is in een andere lidstaat indien de wetgeving van de lidstaat van aansluiting van de verzekerde niet in een aanspraak op die zorg voorziet. Ook mag deze richtlijn de lidstaten niet beletten hun regeling voor verstrekkingen in natura uit te breiden tot in een andere lidstaat verleende gezondheidszorg.
This Directive does not aim to create an entitlement to reimbursement of the costs of healthcare provided in another Member State, if such healthcare is not among the benefits provided for by the legislation of the Member State of affiliation of the insured person. Equally, this Directive should not prevent the Member States from extending their benefits-in-kind scheme to healthcare provided in another Member State.

MEMOREERT de besprekingen tijdens de informele bijeenkomst van de ministers van Volksgezondheid op 28-29 april 2014 te Athene over de economische crisis en de gezondheidszorg, waar de aandacht werd gevestigd op het belang van hervormingen van de gezondheidszorg voor het overwinnen van de crisis en het uitwisselen van beste praktijken en het delen van informatie tussen lidstaten op gebieden van gemeenschappelijk belang, waaronder de kosten van gezondheidszorg, het pakket aan gezondheidszorgdiensten, geneesmiddelen, prestatiebeoordelingen van gezondheidszorgstelsels en investeringen in preventie met het oog op de veerkracht van gezondheidszorgstelsels; er was een brede consensus over de wenselijkheid van het verder toegankelijk maken van gezondheidszorg, met name voor de kwetsbaarste bevolkingsgroepen; het betrekken van de ministers van Volksgezondheid bij het Europees semester kwam eveneens ter sprake.
RECALLS the discussions at the Informal Meeting of Ministers of Health held in Athens on 28-29 April 2014 on the ‘Economic crisis and healthcare’ which highlighted the importance of health reforms to overcome the crisis and exchanging best practices and sharing of information between Member States in areas of common interest including the cost of healthcare, the basket of healthcare services, pharmaceuticals, health systems performance assessment and investing in prevention towards ensuring resilience of health systems; there was broad consensus to improve further access to healthcare particularly for the most vulnerable populations, while the issue of involvement of the Ministers of Health into the European Semester process was also raised.
